2011 - 2012 Player Awards
1.)
To receive award patches for your players, the
process is simple! Print down a
copy of the official awards form,
complete all sections on the form and mail to:
Steve Gesualdi at 7 Fox Run Rd., Danvers MA 01923 along
with a photo copy of the official game sheet. ONLY
ONE patch per award level is allowed per player
per season.
Awarded for three goals in game.

Awarded for three assists in a game.
|
Awarded for a shut-out in a game.

2.) A
player can be listed on the DYH web site an un-limited
number of times. Award postings submitted by noon
on Monday will be posted to the site in that week. Postings
rc'd after noon on Monday will be posted the following
week. To have a name posted on the DYH site, use
the on-line
form. No awards will be posted unless they are submitted
on the form.
